Do nonglycaemic effects such as weight loss account for HbA1c lowering with efpeglenatide?

This study focuses on its impact on HbA1c and other nonglycemic variables in patients with type 2 diabetes (T2DM) and cardiovascular or kidney disease. Over a 1.8-year follow-up, efpeglenatide reduced major adverse cardiovascular events by 27% and lowered HbA1c by 1.24% within 3 months. The trial examined whether changes in variables like weight, cholesterol levels, and hepatic function mediated the drug's glycemic effects. Results show that while efpeglenatide improved multiple nonglycemic measures, its HbA1c-lowering effect was largely independent of these changes, with a modest 11.7% attenuation when adjusted for mediating variables.
