ONCOLOGY

The use of ibrutinib in the treatment of chronic lymphocytic leukemia (CLL) has significantly changed the treatment approach. Clinical trials such as RESONATE-2 and iLLuminate have demonstrated the superiority of ibrutinib over traditional therapies like chlorambucil. However, there have been concerns about the choice of chlorambucil as a control arm in these trials. A retrospective analysis of SEER-Medicare data revealed that the use of chlorambucil was limited to older patients with higher comorbidity burdens. Prior to the introduction of ibrutinib, more aggressive therapies like bendamustine and rituximab were commonly used in CLL treatment. The study suggests that the choice of control treatment can greatly impact clinical trial results and highlights the importance of considering standard care practices in future trials.

Most recommendations remain conditional because evidence certainty is low or very low in several clinical areas.
