Donor-Recipient Sex Matching Impacts Liver Transplant Survival

A European Liver Transplant Registry analysis of 7601 patients undergoing liver transplantation (LT) for hepatocellular carcinoma (HCC) revealed that donor-recipient sex constellation (RDSC) impacts long-term survival. Median follow-up was 22.6 months, with 25.1% mortality and 26.0% attributed to HCC recurrence. While crude survival estimates showed no differences between RDSC groups, multivariable analysis indicated that male donor female recipient (MDFR) transplants were associated with a 28% lower mortality risk compared to female donor female recipient (FDFR) transplants (aHR 0.72, p = 0.023). Other RDSC groups showed no significant differences in mortality risk. Differences were noted in rejection and cardiovascular-related deaths across RDSC groups. These findings suggest that donor sex may influence survival outcomes for female recipients of LT for HCC.
