
A 7-year-old girl presented with a perianal lesion and behavioral changes, leading to a diagnosis of syphilis and suspected sexual abuse. Further history revealed untreated oral and vaginal ulcers, raising concerns. Positive VDRL and TPHA results confirmed the diagnosis, and a medicolegal case was initiated. Post-traumatic stress disorder was diagnosed, and counseling was recommended. Benzathine penicillin treatment improved symptoms, emphasizing the critical role of pediatricians in identifying abuse.
