
A study assessed the effects of early coronary revascularization (<24 hours) versus an initial conservative approach on clinical outcomes in diabetic patients with non-ST elevation acute coronary syndrome (NSTE-ACS). Analyzing data from 7,589 patients, including 2,343 diabetics, the study found that elderly patients (>65 years) who underwent early revascularization had lower 30-day mortality compared to those receiving conservative treatment (3.3% vs. 6.7%; RR: 0.48). In contrast, younger patients did not show a survival benefit from early revascularization. Diabetes was a significant mortality predictor in the elderly (OR: 1.43), but not in younger patients. The findings suggest that early revascularization may be more beneficial for elderly diabetic patients, while younger patients might not experience the same advantage.
