The study analyzed 29 randomized controlled trials involving 2045 men with idiopathic oligo-astheno-teratozoospermia (OAT). Among 19 antioxidant preparations, l-carnitine, particularly when combined with l-acetyl-carnitine (LAC), showed the most significant improvement in sperm concentration, progressive motility, and morphology. Vitamin E+selenium or zinc were most effective for total motility, while pentoxifylline and vitamin E+CoQ10 contributed to progressive motility.
