A study examined the effects of pioglitazone and empagliflozin on patients with type 2 diabetes mellitus (T2DM) and nonalcoholic fatty liver disease (NAFLD), a condition linked to cardiovascular risks. Global longitudinal strain (GLS), a measure of left ventricular mechanics, was used to detect subclinical myocardial issues. Both drugs improved GLS significantly without a notable difference between them. Liver stiffness also decreased in both groups. Empagliflozin showed better results in improving liver steatosis. These findings highlight the importance of addressing cardiac dysfunction in high-risk T2DM patients with NAFLD and suggest potential prognostic implications
