This study investigated the effects of intracanal cryotherapy on biomarker levels and pain management in mandibular pre-molar teeth with asymptomatic apical periodontitis. Forty-four patients received root canal treatment and were divided into control and cryotherapy groups. Periapical exudate samples were collected before and after treatment, and biomarker levels were measured. The results showed a correlation between pain scores and IL-1β and PGE2 levels. Cryotherapy effectively reduced post-operative pain for the first three days. It also prevented an increase in IL-1β, IL-2, and IL-6 levels compared to the control group. These findings suggest that cryotherapy can be beneficial for managing pain in asymptomatic apical periodontitis.
