The NUDGE-FLU trial aimed to evaluate the effectiveness of electronic behavioral nudging letters in increasing influenza vaccination rates in patients with heart failure (HF) in Denmark. The trial involved 964,870 Danish citizens aged 65 and above, who received different nudging letter strategies. The primary endpoint was influenza vaccine receipt, and secondary outcomes included the use of guideline-directed medical therapy (GDMT). The analysis revealed that influenza vaccination uptake in the overall Danish HF population was 71.6%, but only 44.6% in those under 65. Higher baseline GDMT use was associated with higher vaccination rates. HF status did not affect the effectiveness of the nudging strategies, and no negative effects on GDMT use were observed. However, a significant implementation gap existed in vaccinating HF patients.
