Enhancing Our Understanding of Breathing Mechanics in Nonintubated Patients with Acute Hypoxemic Respiratory Failure

Advancements in understanding and managing acute hypoxemic respiratory failure (AHRF) have been made in the past 50 years. Noninvasive strategies such as high-flow nasal oxygen (HFNO) and helmet interfaces have emerged as options to avoid endotracheal intubation. However, even noninvasive strategies have potential risks, including ventilator-induced lung injury. A study evaluated three noninvasive strategies (helmet pressure support, helmet continuous positive airway pressure (CPAP), and HFNO) in patients with severe AHRF. The use of helmet pressure support reduced inspiratory effort and improved oxygenation compared to CPAP or HFNO. Helmet interfaces were associated with higher tidal volumes and end-expiratory lung volumes compared to HFNO. Individual patient assessment and recognizing when noninvasive approaches fail are crucial for successful management.
