The study found that a reduced erector spinae muscle cross-sectional area (ESMCSA) significantly correlates with poor lung function and higher 5-year all-cause mortality in COPD patients. An ESMCSA cutoff of 23.42 cm² predicted mortality risk, and multivariate analysis confirmed ESMCSA as an independent predictor. A nomogram incorporating ESMCSA showed high accuracy (C-index: 0.852–0.890), offering a valuable tool for prognosis.
