A recent Keynote-522 study highlights the efficacy of pembrolizumab alongside chemotherapy in treating high-risk, early-stage triple-negative breast cancer (TNBC). The study focused on the prognostic value of the residual cancer burden (RCB) index in assessing treatment outcomes. Results indicate that pembrolizumab reduced RCB categories in patients not achieving complete response. This reduction correlated with improved event-free survival (EFS) in the RCB-2 category. Notably, pembrolizumab also decreased distant recurrences, with a higher proportion of central nervous system recurrences in the placebo group. These findings emphasize pembrolizumab's dual benefit in enhancing pathological complete response rates and improving EFS, particularly in non-pCR patients.
