Existing Nongated CT Coronary Calcium Predicts Operative Risk in Patients Undergoing Noncardiac Surgeries (ENCORES)

Preoperative risk assessment for noncardiac surgery is enhanced by evaluating coronary calcium from previous nongated chest CT scans. Researchers found that higher coronary calcium scores were linked to increased perioperative major clinical events (MCE), such as death or heart attacks within 30 days of surgery. In a study involving 2,554 adults, an estimated coronary calcium burden (ECCB) score of ≥3 was associated with a two-fold higher risk of MCE than ECCB <3. This method improved risk prediction beyond the Revised Cardiac Risk Index. The findings suggest that existing chest CT scans can aid in better preoperative risk stratification.
