The study investigates using extracellular vesicles (EVs) to predict tumor response to neoadjuvant chemotherapy (NAC) in breast cancer. Drug-resistant triple-negative breast cancer cell lines were established and EVs isolated from them. Drug efflux transporters were highly expressed in resistant cell lines and EVs. Patients with residual disease showed higher expression of drug resistance markers in EVs. A combination of these markers predicted NAC resistance with high sensitivity (81.82%) and specificity (92.86%). EVs offer promise for assessing tumor response to NAC.
