
In a propensity-matched cohort of 976 gout patients, febuxostat users showed a hepatotoxicity incidence of 39.6 per 1,000 person-years versus 16.8 for benzbromarone. Adjusted hazard ratio was 2.75 (95% CI: 1.28–5.91). Elevated baseline liver enzymes increased risk further. Subgroup analyses revealed consistent findings, confirming febuxostat’s association with a significantly higher risk of liver enzyme elevation.
