Using data from 10,544 patients (2005–2023), this nationwide German study compares perioperative complications among focal therapies (FT) for prostate cancer. HIFU was most common (92%) and had higher urinary tract infection rates but lower ICU admissions than other FT methods. Complications were fewer with FT than with robotic surgery, but more than with brachytherapy. TURP increased risks significantly. Overall, FT showed a low complication profile.
