Frozen Section Histology’s Role in HPV-Related OPSCC Surgical Margins

Intraoperative frozen section histology (IFSH) shows high specificity (99.1%) but low sensitivity (21.7%) in assessing final surgical margin status in HPV-related oropharyngeal squamous cell carcinoma (OPSCC). While it has limited accuracy in detecting positive margins, IFSH is valuable in guiding reresection, with 27% of patients benefiting from this. Despite its limitations, IFSH contributes to improved oncologic outcomes, especially by identifying patients without disease through negative readings.
