The study evaluated functional and anatomical outcomes of fungal endophthalmitis following cataract surgery at King Khaled Eye Specialist Hospital. A retrospective analysis included 29 patients diagnosed with post-cataract surgery endophthalmitis between January 2017 and December 2022. The mean time from surgery to symptom onset was 52.1 days. Low culture positivity was noted in four samples, with Aspergillus and Cladosporium identified. Six patients underwent pars plana vitrectomy (PPV) with intravitreal antimicrobial injections, resulting in functional success in 50% and anatomical success in 83.3%. In contrast, the conservative treatment group achieved functional success in 43.5% and anatomical success in 69.6%.
