This systematic review and meta-analysis examined the relationship between gestational diabetes mellitus (GDM) and neonatal respiratory distress syndrome (NRDS). A total of 44 studies, involving over 6.2 million participants, were included. The analysis revealed a significant association between GDM and an increased risk of NRDS in newborns (OR 1.9; 95% CI 1.5–2.3). Subgroup analysis indicated that this relationship held for both term and preterm pregnancies, across various regions, and in both GDM-based and NRDS-based studies. Sensitivity analysis confirmed the robustness of these findings. The study underscores the importance of early GDM detection and management to mitigate the risk of adverse neonatal outcomes. However, factors like high heterogeneity and publication bias must be addressed in future studies to refine these conclusions and explore preventive strategies.
