The rising demand for GLP-1 receptor agonists (GLP-1 RAs) in managing diabetes mellitus (DM) and obesity has outpaced supply and is hindered by high costs, limiting accessibility. A narrative review highlighted ongoing advancements in GLP-1 RA biosimilars, particularly liraglutide and semaglutide. Preliminary data on liraglutide biosimilars show comparable efficacy and safety to reference products. Semaglutide and beinaglutide biosimilars are under investigation, aiming to address accessibility issues. Current out-of-pocket monthly costs for liraglutide, semaglutide, and tirzepatide range from $892 to $1418, presenting challenges for DM, obesity, and related conditions like obstructive sleep apnoea and non-alcoholic fatty liver disease.
