
In a recent study, researchers investigated the intricacies of sarcoid neuropathy, uncovering potential mechanisms behind the condition. The study analyzed 18 patients, and electrophysiological assessments revealed axonal neuropathy indications while nerve conduction abnormalities were prevalent. Notably, 11 patients displayed a conduction block, with five meeting chronic inflammatory demyelinating polyneuropathy (CIDP) criteria. Sural nerve biopsies indicated myelinated fiber loss and demyelinated axons near granulomas. These findings suggest a significant role of demyelination alongside ischemic damage in the pathogenesis of sarcoid neuropathy, potentially altering the understanding and treatment of the condition.
