Guideline changes improved aspirin use in Black patients, but gaps remain. A study of 31,555 pregnancies found an 8.8 percentage-point increase in low-dose aspirin use among Black patients after 2021 USPSTF/ACOG updates, but no increase among Medicaid recipients. Despite new recommendations, aspirin use remains below expected levels. Further efforts are needed to enhance adherence and address disparities in preeclampsia prevention.
