
A recent review delves into the mechanisms underlying High-Altitude Pulmonary Edema (HAPE), a condition caused by hypoxia at high altitudes. While current understanding attributes the pathophysiology to hypoxic pulmonary vasoconstriction, this study explores additional cellular mechanisms within the acinar area. The review highlights the role of various factors connecting cells within the alveolo-capillary barrier, including damage to epithelial cells, increased endothelial permeability, inflammation triggered by macrophages, disruption of extracellular matrix and tight junctions, and orchestrated responses of endothelial and smooth muscle cells. Hypoxia also affects fibroblast and pericyte cells, further impacting the alveolar-capillary barrier. These complex interactions ultimately lead to rapid water accumulation in the alveoli.
