
Recent research challenges the notion that high levels of high-density lipoprotein cholesterol (HDL-C) offer cardiovascular protection, suggesting a potential association with worsened cardiovascular disease (CVD) outcomes. This study investigated HDL-C's link to interstitial myocardial fibrosis (IMF) and myocardial scar using cardiac magnetic resonance imaging in 1863 participants with a mean age of 69. The analysis revealed that higher HDL-C levels are associated with increased markers of IMF, particularly in individuals with HDL-C levels ≥60mg/dL. However, HDL-C levels show no significant association with the prevalence of myocardial scars. These findings underscore the complexity of HDL-C's role in cardiovascular health.
