The prospective cohort study explored the association between visceral fat-to-muscle ratio (VMR) and severe COPD exacerbations requiring hospitalization. Conducted between May 2018 and December 2023, the study included 631 COPD patients. The results revealed that VMR had a stronger association with severe exacerbations than body mass index and other obesity indicators. A higher VMR increased the risk of 1-year and 5-year exacerbations. Subgroup analysis showed stronger associations in females and overweight individuals. These findings suggest that VMR could be a useful risk factor for managing COPD exacerbations.
