Higher Bleeding Risk in Dual Therapy Dental Patients

This retrospective study analyzed 1595 dental extraction procedures to identify high-risk patients on antiplatelet (AP) or anticoagulant therapy and the timing of postoperative hemorrhage. Patients were categorized into monotherapy groups (AP, indirect oral anticoagulants [IAC], direct oral anticoagulants [DOAC]) and dual therapy groups (double AP, AP + IAC, AP + DOAC). Dual therapy groups exhibited a significantly higher incidence of hemorrhagic events compared to monotherapy (p < 0.001). Among monotherapy groups, DOAC patients had the highest postoperative bleeding risk and increased repeated bleeding episodes. Most bleeding incidents occurred on the day of surgery (44%-51.1%). While the overall bleeding risk was low, preventive hospitalization was often driven by comorbidities rather than bleeding risks.
