
This pilot study investigates the role of perfusate hemoglobin in normothermic machine perfusion (NMP) of donor livers and its association with early allograft dysfunction (EAD). Among 15 donor livers, higher initial perfusate hemoglobin was linked to reduced EAD risk and lower peak transaminase levels in recipients. The decrease in perfusate hemoglobin during NMP correlated with liver injury markers. These findings suggest that maintaining higher hemoglobin during NMP may help reduce ischemia-reperfusion injury and improve outcomes.
