Hope for Monochorionic Twin Pregnancies: Laser Therapy vs. Expectant Management

The review compares fetoscopic laser photocoagulation with expectant management for selective fetal growth restriction (sFGR) in monochorionic twins when cord coagulation is not an option. Six studies involving 299 pregnancies were included, and the results showed that laser is associated with more fetal deaths of the FGR twin compared to expectant management, without altering gestational age at delivery or rates of neonatal death. However, laser was associated with decreased abnormal neuroimaging in the AGA twin. The data is limited, and randomized trials with long-term neurological outcomes are needed to address laser for type II sFGR.
