The study examined the relationship between intracranial pressure (ICP) pulse waveform and computed tomography (CT) features in traumatic brain injury (TBI) patients. A neural network classified ICP pulses into four classes, and the pulse shape index (PSI) was calculated. PSI was significantly higher in patients with mass lesions or midline shift, showing a correlation with CT measures. PSI outperformed mean ICP and pulse amplitude of ICP (AmpICP) in identifying pathological CT features. The findings suggest that the ICP pulse shape reflects reduced cerebrospinal compensatory reserve, emphasizing its potential as a monitoring strategy in TBI patients.
