IGFBP-1 predicts and drives AKI severity post-cardiac surgery. In this study, urinary IGFBP-1 emerged as a strong biomarker for predicting progression from mild to severe acute kidney injury (AKI). Validation in human cohorts and functional studies in a murine model of cisplatin-induced AKI confirmed its pathogenic role. Targeting IGFBP-1 may offer both prognostic and therapeutic value in managing AKI.
