The Ikelos-Rating Scale (Ikelos-RS) is a reliable tool for iRBD. Validated in 180 patients, Ikelos-RS showed high inter-rater (rs = 0.865) and test–retest reliability (rs = 0.900). It demonstrated construct validity with strong correlations to clinical severity (CGI-S), REM sleep without atonia (RWA), and DaT-SPECT scans. Ikelos-RS is a promising tool for assessing iRBD severity and may be beneficial for research, pending further studies on its use in screening.
