IL-12 and IL-23 Pathway Inhibition as Promising Therapeutic Strategies in Inflammatory Bowel Disease

Interleukin-12 (IL-12) and interleukin-23 (IL-23) are cytokines that belong to the IL-12 family and play a critical role in maintaining intestinal homeostasis and inflammation, and are implicated in the pathogenesis of inflammatory bowel disease (IBD). They are secreted by antigen-presenting cells and have pro-inflammatory and anti-inflammatory effects through receptor-mediated mechanisms. Increased understanding of the pro-inflammatory effects of IL-12 and IL-23 has led to the development of monoclonal antibodies that target the shared subunit (p40) of IL-12 and IL-23, such as ustekinumab and briakinumab, or the IL-23-specific subunit (p19), including risankizumab, guselkumab, brazikumab, and mirikizumab. This review highlights the development of IL-12- and IL-23-targeted drugs for treating Crohn's disease and ulcerative colitis.
