Imlunestrant and Abemaciclib Boost Outcomes in Breast Cancer

Imlunestrant, a novel oral selective estrogen receptor degrader, was evaluated in the phase III EMBER-3 trial for ER-positive, HER2-negative advanced breast cancer. Among 874 randomized patients, imlunestrant demonstrated superior progression-free survival (PFS) compared to standard endocrine therapy in those with ESR1 mutations (median PFS: 5.5 vs. 3.8 months; P<0.001). In the overall population, PFS did not differ significantly between imlunestrant and standard therapy. However, imlunestrant–abemaciclib significantly improved PFS over imlunestrant alone (9.4 vs. 5.5 months; P<0.001). Grade ≥3 adverse events occurred in 17.1% of patients on imlunestrant, 20.7% on standard therapy, and 48.6% on imlunestrant–abemaciclib.
