This pilot study aimed to compare the expression of tryptase and cyclooxygenase-2 (COX-2) in periapical granuloma (PG) and periapical cyst (PC) and examine their correlation with inflammatory infiltrate intensity and cystic epithelial lining thickness. Tissue samples from 50 PGs and 50 PCs were analyzed using immunohistochemical methods. The results showed similar inflammatory scores, total mast cell counts, and COX-2 expression in PGs and PCs. However, degranulated mast cells were significantly higher in PCs, while granulated mast cells were significantly higher in PGs. There was a correlation between total mast cells and epithelial thickness in PCs. Mast cells and COX-2 may serve as independent inflammatory markers in periapical lesions, suggesting potential treatment options involving mast cell and COX-2 inhibitors. Further research is needed in this area.
