The study analyzed randomized, placebo-controlled trials to determine if the benefits of sodium-glucose co-transporter-2 inhibitors (SGLT2is) for cardiorenal outcomes varied with baseline kidney function. The study found that SGLT2is reduced the risk of renal disease progression and heart failure outcomes for all estimated glomerular filtration rate (eGFR) categories, with the greatest benefit for kidney protection observed in those with preserved eGFR and the greatest benefit for heart failure outcomes observed in those with lower eGFR values. Meta-regression analysis showed a direct relationship between baseline eGFR and the magnitude of SGLT2is' renal protection. Overall, early initiation of SGLT2is in people with preserved eGFR may achieve the greatest benefits in terms of kidney protection.
