Donor characteristics significantly influence transplant outcomes. Low CD34+ cell dose worsens survival after PTCy-haplo, while older donor age and HLA mismatches reduce survival after ATG-haplo. Older donors also have a higher risk of acute GVHD. Offspring donors have better relapse-free survival and GVHD-free relapse-free survival than siblings. These findings emphasize the importance of donor selection in optimizing TCR haploidentical PBSCT outcomes with PTCy or ATG.
