This retrospective cohort study assessed the relationship between body mass index (BMI) and severe maternal morbidity (SMM) using data from 223,837 singleton live births. Results indicated that an increase in BMI was associated with a higher risk of SMM and maternal ICU admission. For each 1-point increase in BMI, the odds of SMM increased by 0.8%, and ICU admission by 2.0%. Obese patients (BMI ≥40 kg/m²) had significantly higher odds of ICU admission, with those having BMI ≥50 kg/m² showing a 300% increase in risk. These findings suggest that higher BMI is a risk factor for SMM and ICU admission.
