Impaired Suppression of CD4+ and CD8+ TRMs Endangers Melanocyte Survival in Generalized Vitiligo

The study investigated Regulatory T cells (Tregs) in generalized vitiligo (GV), focusing on resident memory (TRM)-Tregs and antigen-specific Tregs in patients. GV patients exhibited elevated CD4+ & CD8+ TRM cells in skin layers compared to controls, while TRM-Tregs and antigen-specific Tregs were reduced. This imbalance led to decreased CD4+ and CD8+ TRM cell suppression, marked by increased IFN-γ, perforin, and granzyme B. Additionally, impaired production of immunosuppressive cytokines (IL-10 & TGF-β) and heightened expression of TRM-stimulating cytokines (IL-15 & IL-17A) were observed. These findings suggest compromised melanocyte survival in GV due to dysfunctional TRM-Tregs and antigen-specific Tregs.
