
This review aimed to investigate the evidence regarding the role of implant connection and abutment characteristics in predisposing or precipitating peri-implant mucositis and peri-implantitis. Although the evidence is limited, studies suggest that implant connection, trans-mucosal configuration, and handling may play a potential role in developing early bone loss or peri-implantitis. Conical internal connections and tissue-level implants may decrease the risk of early bone loss and peri-implant disease, and biocompatible materials such as titanium or zirconia should be preferred in the trans-mucosal portion. Longer implants with an emergence angle below 30° seem more favorable, but more research is needed before making firm recommendations about abutment choice.
