The study aimed to determine the incidence of remission and relapse in patients with type 2 diabetes and identify associated factors. A total of 48,320 Japanese patients with type 2 diabetes were followed from 1989 to September 2022. Remission was defined as having a glycated haemoglobin (HbA1c) level below 48 mmol/mol after stopping glucose-lowering drugs for at least 3 months, while relapse was defined as failing to maintain remission for 1 year. Factors such as baseline HbA1c, body mass index (BMI), and duration of diabetes were evaluated. The incidence of remission was 10.5 per 1000 person-years, with higher rates observed in patients with specific characteristics. Among those who achieved remission, two-thirds relapsed within a year. The study highlighted differences in remission incidence and relapse predictors, particularly related to BMI, between East Asian and Western populations, suggesting ethnic variations in returning to normal glucose levels from hyperglycemia.
