
This retrospective cohort study aimed to examine the outcomes of posttransplant lymphoproliferative disorder (PTLD), graft loss, and patient mortality in adult kidney transplant recipients based on their Epstein–Barr virus (EBV) donor and recipient serostatus. The study found that EBV D+/R− recipients had a significantly higher cumulative incidence of PTLD (22.1%) compared to EBV R+ recipients. The incidence of graft failure was also higher in EBV D+/R− recipients, with a hazard ratio of 2.21, indicating a 5- to 10-fold increased risk compared to previous studies. The findings suggest the need for strategies to mitigate the risk of PTLD in these patients.
