MRI identified a hyperacute infarct, and thrombolysis with alteplase was initiated. Following thrombolysis, the patient experienced a parenchymal hemorrhage. Further evaluation revealed infective endocarditis related to previously undiagnosed mitral valve prolapse as the cause of stroke. This case underscores the need for emergency physicians and intensivists to consider infectious etiologies, especially in febrile young stroke patients, as thrombolysis could result in adverse outcomes.
