The randomized study compared intensive sleep retraining (ISR), total sleep deprivation (TSD), and a control condition in adults with chronic insomnia. ISR and TSD significantly reduced insomnia severity and fatigue, but only ISR significantly improved sleep onset latency. These effects were maintained at 3 months, supporting ISR's dual mechanisms: resolving conditioned insomnia and enhancing sleep drive through deprivation.
