
Inducible tertiary lymphoid structures (iTLSs) drive OLP severity. Single-cell RNA sequencing and multiplex immunofluorescence revealed fibroblast-immune interactions shaping iTLSs. CCL19+ fibroblasts recruit T cells, with lymphotoxin activating fibroblasts. Patients with iTLSs showed more severe OLP symptoms. The findings suggest iTLSs as key regulators of adaptive immunity in oral lichen planus, influencing disease progression.
