Left ventricular lead placement in cardiac resynchronization therapy

This paper provides a review of literature on image-guided cardiac resynchronization therapy (CRT) studies. The focus is on the association between improved CRT outcomes and the placement of a left ventricular (LV) lead in the latest activated segment free from scar. Despite this association, most randomized clinical trials exploring guided LV lead implantation did not demonstrate superiority over conventional approaches. The review discusses potential factors contributing to this paradoxical observation, such as inclusion criteria, activation wavefront differences, target region definition, and limitations in coronary venous anatomy.
