A recent study examined 564 patients who underwent nephrectomy for localized sarcomatoid renal cell carcinoma (sRCC) and Grade 4 renal cell carcinoma (RCC). The findings revealed that sRCC patients had significantly worse cancer-specific survival (CSS) outcomes, even in early stages. Poor CSS predictors included sarcomatoid features, non-clear cell histology, positive margins, higher stage, and minimally invasive surgery (MIS). These results highlight the urgent need for close monitoring and consideration of sRCC patients in adjuvant therapy trials, as this aggressive cancer subtype has a grim prognosis, even in localized cases. The study's external validation using ASSURE clinical trial data confirmed these alarming disparities in CSS.
