Long-Term Effects of Pulmonary Endarterectomy on Right Ventricular Stiffness and Fibrosis in Chronic Thromboembolic Pulmonary Hypertension

After pulmonary endarterectomy (PEA), a chronic thromboembolic pulmonary hypertension procedure, right ventricular (RV) function typically improves, but RV mass often remains abnormal. This study involving 25 patients examined the causes of this incomplete reversal of RV remodeling. It found that while cellular hypertrophy and diastolic stiffness improved, there was still an increase in extracellular volume, indicating persistent interstitial fibrosis. Elevated levels of collagen-related biomarkers suggest ongoing collagen turnover. These findings shed light on the complexities of RV remodeling post-PEA and may inform future treatment strategies.
