The 30-year Swedish twin study found that lower fasting IGFBP-1 at baseline predicted higher BMI (by 1.8 kg/m²) and WHtR (by 3 cm/m) at age 73. IGF-I showed no association. These links weakened with insulin adjustment and didn’t predict survival. Familial factors partly explained results. IGFBP-1 is a consistent marker of adiposity, not of weight change or mortality.
