Lung Recruitment Assessed by Electrical Impedance Tomography (RECRUIT): A Multicenter Study of COVID-19 Acute Respiratory Distress Syndrome

A study has found that lung recruitability, a crucial factor in selecting the appropriate positive end-expiratory pressure (PEEP) for ventilated patients, can vary significantly among individuals with COVID-19. The research utilized electrical impedance tomography (EIT) to assess recruitability, respiratory mechanics, and gas exchange during PEEP titration. Optimal EIT-based PEEP levels were determined by identifying the point where overdistension and collapse curves intersected. The study classified patients as low, medium, or high recruiters based on the extent of modifiable collapse. Personalizing PEEP based on recruitability and overdistension could differ significantly from traditional approaches, providing a safer and more effective method for mechanical ventilation.
