
Mannitol provocation improves the laryngoscopic diagnosis of inducible laryngeal obstruction (ILO) by accentuating paradoxical vocal fold movement (PVFM). Among 80 suspected ILO patients, PVFM increased from 52.5% at baseline to 72.5% post-mannitol. Accentuation occurred in 56% of cases but was absent in healthy volunteers. Isolated expiratory PVFM without accentuation was not linked to airway obstruction or bronchial hyperresponsiveness.
